Abstract

Objective. To analyze the cosmetic surgery impact on the breast in breastfeeding.
Discussion and results. Augmentation mammoplasty: Some studies evaluating the safety and efficacy of devices implanted through inframammary and periareolar incision sites, resulted in a low risk of changes in nipple or skin sensation and breastfeeding problems, and data are reported long-term safety and effectiveness. On the other hand, several investigations found that there is a higher risk of pain, insufficient breastfeeding and more frequent use of oral galactogogues in women with retro-glandural implants and with larger implants. Regarding the submammary and areolar approach, in the latter, it was found that there is a greater probability of damaging some nerve ending in the breast and the ducts, affecting the sensitivity of the breast and inhibiting milk production. Reduction mammoplasty: Research agrees that breast reduction surgery preserving the pedicle is the one that most guarantees the innervation preservation and therefore the nipple sensitivity , as well as the integrity of the ducts. In addition, there is up to 5 times more risk of insufficient breastfeeding among women undergoing this type of surgery.
Conclusions. In general, diffuse and heterogeneous information is observed regarding the impact of breast surgeries on breastfeeding. Regarding augmentation mammoplasty, in general, the risk of changes in nipple sensation and breastfeeding problems are low and provide long-term safety and effectiveness. However, there are studies that show that the incidence rate of exclusive breastfeeding is lower. In addition, there is a greater risk of pain and insufficient breastfeeding with retroglandular implants than with retromuscular implants, with larger implants and by an areolar approach than inframammary. Regarding reduction surgery, it has been found that it has the most negative impact on breastfeeding.
